Jotform is the best forms app I have ever tried. It is not just that is simple to sue. It can also get as complex as you want and gives us the potential to create almost any workflow around the forms. It is the best form solution in the market right now

Flexible & Powerful Smart-Form Generator
I've been using Jotform for years and have used it for a variety of functions including registration for events, memberships, surveys and more. I like that I can customize the form entry experience to make it streamlined and more usable. Also, customizing the auto-responder based on submission details is also really helpful.

Abysmal support and bad HIPAA implementation
We are a small urgent care that started using JotForm last year to reduce our paper forms during the pandemic. The platform is actually quite robust and user-friendly -- but PRAY that you never have an issue that requires using their customer support. No phone, no live chat -- online support forum only. It's absolutely pointless for an urgent issue, since you never get a response sooner than 3-4 hours after you post. The support staff is only capable of parroting information that is ...

Very affordable and simple to use
