I've just use Jotform yesterday and it's quite useful for me. However, if you use a free account, you should reconsider because of its limited submissions per month

Used the Jotform for the past 8months…
Used the Jotform for the past 8months or so. It is very easy to use and interactive, support is great when needed. The pricing is very affordable as well. We use this for a variety of areas, in safety checks and inspections and voting forms for fun & newsletters for employees too.
