jotform allows for seamless integration while keeping things super simple, you can have as many questions as you want and they have tons of free templates to choose from would recomend to a freind

Within 4 hours: Problem SOLVED!
I had a small technical glitch I couldn't figure out for the life of me. Sent 1 help request to Jotform, got a response back within 4 hours, and BAM: it was totally resolved. I love this kind of service!

Used the Jotform for the past 8months…
Used the Jotform for the past 8months or so. It is very easy to use and interactive, support is great when needed. The pricing is very affordable as well. We use this for a variety of areas, in safety checks and inspections and voting forms for fun & newsletters for employees too.
