With Covid I needed to create a form…
With Covid I needed to create a form that our patients could easily fill out prior to their dental appointment. Tried google docs, that was to complicated and we thought DocuSign, to expensive. Jot Form filled our needs. Highly recommend Jot Form for all your needs.

A nice way to create questionnaires
A nice way to create questionnaires, registration forms and feedback forms. I use it mainly for this 3 thins as an completion aid! It's very easy to create and you can design it very individually.
